University of Rhode Island-Alpha Phi was installed in 1999 (I was one of them!)
Bentley College (MA) only has informal in the fall

So you can update your stats...the Alpha Epsilon (Duke U.) chapter of AEPhi went dormant in 2004.

Alpha Gamma Delta at Michigan State University closed in 2002. (This was my chapter of initiation. )
This list is awesome! I'm so impressed - many things I've been curious about (but not able to look up as I don't have access to Baird's) have been answered.
ETA: Bowling Green State University (Ohio) recruitment dates are Sept 6-10. (I'm chapter advisor there - I'll keep you posted on results in the fall...)

Just a note, Sigma Sigma Sigma is no longer at North Carolina Wesleyan College, the chapter closed in 2006.

Just a note: Tri-Sigma at Indiana University Bloomington closed in 1994. (I was there at the time - there was also a big feature in the 1993-1994 yearbook about the chapter closing) I think ASA came to campus in 1989 or 1990. (their first formal rush was 1990-91 per an unofficial IU guidebook I have)
Also, here are the dates of IU's formal rush, per the PHA website:

Some more Duke historical info
Kappa Delta was active at Duke between 1912-1967, and again from 1976-1990
Zeta Tau Alpha was active at Duke between 1915-1995, and again from 2005-present
Alpha Phi: This one's a bit weird. The web site for Duke's Office of Fraternity and Sorority Life says that this group's active dates are 1935-2003 and 2003-present. I know for a fact that when I was on campus in the mid-80s Alpha Phi was not there, but the 2003-present date seems correct. Maybe someone else knows if Alpha Phi existed at Duke way back when?

I'm going to assume the website is incorrect since I doubt they closed and re-opened in the same year.

My 1991 Baird's gives Alpha Phi's dates at Duke from 1935 to 1970.
